MAPLE GROVE – 2013 PARK IMPROVEMENTS PROJECT

Sealed bids will be received by the City of Maple Grove, Minnesota, in the City Hall at 12800 Arbor Lakes Parkway until 4:00 p.m. on May 1, 2013, at which place and time they will be publicly opened by two or more persons who have been designated by the City to open bids.
Bids arriving after the designated time will be returned unopened.
Bids are for the furnishing of all labor and materials for the construction of two parks in accordance with the plans and specifications, consisting primarily of the following estimated quantities:
Maple Creek Neighborhood Park:
This 5.5 acre site is located in southwest Maple Grove.  Rough grading of the site is being completed under separate contract in cooperation with the adjacent housing development.  A small parking lot is being constructed under separate contract through the adjacent street construction project.  Major work items include:  Ball field construction including fine grading, chainlink fencing, backstops, aglime surfacing and concrete surfacing.  Other site improvements include a basketball court, concrete paving, asphalt trail paving, landscaping, turf establishment, site furniture, retaining walls, and other miscellaneous improvements.
Balsam Play Lot:
This site is located within the Donegal Subdivision in the gravel mining area of the City.  Rough grading of the site is being completed under separate contract in cooperation with the surrounding housing development.  Major work items will include:  Concrete paving, asphalt trail paving, seeding and sodding, landscaping, half basketball court construction, site furnishings, retaining walls, and other miscellaneous improvements.
Both Parks are located within the city of Maple Grove, Minnesota.
Plans and specifications, proposal forms and contract documents may be seen at the office of the City Clerk, Maple Grove, Minnesota.
All bids shall be sealed and marked “Maple Grove 2013 Park Improvements Project” and shall be accompanied by a bidder’s bond naming the City of Maple Grove as obligee, certified check payable to the Clerk of the City of Maple Grove or a cash deposit equal to at least five percent (5%) of the amount of the bid, which shall be forfeited to the City in the event that the bidder fails to enter into a contract.
The City Council reserves the right to retain the deposits of the three lowest bidders for a period not to exceed thirty (30) days after the date and time set for the opening of bids. No bids may be withdrawn for a period of thirty (30) days after the date and time set for the opening of bids.
Payment for the work will be by cash or check.
Bids shall be on the forms provided for that purpose and according to the contract documents prepared by the skatepark architect and the City of Maple Grove.

All bidders for this contract, including subcontractors and suppliers that have 40 or more full-time employees, shall submit a certified copy of their current Affirmative Action Certificate with their bid.
The Council reserves the right to reject any and all bids, to waive irregularities and informalities therein and further reserves the right to award the contract to the best interests of the City.

2013 Alley Improvements
TO WHOM IT MAY CONCERN:
Notice is hereby given that the City Council of Osseo will meet in the Council Chambers of the City Hall at 7:00 p.m. on Monday, April 22, 2013, to consider the making of an improvement on the alley located between 3rd Avenue NE and 4th Avenue NE and running from 4th Street Northeast to 93rd Avenue (County Road 30). Full reconstruction of the alley pavement is proposed.
The improvements will be done pursuant to Minnesota Statute §§ 429.011 to 429.111. The area proposed to be assessed for such improvements are those properties which abut the proposed improvement. The total estimated cost of the improvement is $109,000. A reasonable estimate of the impact of the assessment will be available at the hearing. Such persons as desire to be heard with reference to the proposed improvement will be heard at this meeting.

NOTICE OF INTENT TO REQUEST RELEASE OF FUNDS

TO ALL INTERESTED PARTIES, GROUPS AND PERSONS:
The purpose of this notice is to identify two separate but related actions to be undertaken by Hennepin County. On or about May 6, 2013, Hennepin County will request that the U.S. Department of Housing and Urban Development (HUD) to release HOME Investment Partnerships Program (HOME) funds under Title II of the National Affordable Housing Act to undertake a project known as Bottineau Ridge Apartments as described below:
Duffy Development, on behalf of the to be created partnership, will acquire property at the intersection of Hemlock Lane and 80th Street N. in the city of Maple Grove to construct Bottineau Ridge Apartments. Bottineau Ridge will be a three-story, 50 unit building with a mix of one, two and three bedrooms and underground parking. All units will be affordable to families with incomes at or below 50% of the Area Median Income (AMI). The project will have a total development cost of approximately, $10.5 million, which will include up to $750,000 of HOME funds.
FINDING OF NO SIGNIFICANT IMPACT
It has been determined that such a request for release of funds will not constitute an activity significantly affecting the quality of the human environment and, accordingly, Hennepin County has decided not to prepare an Environmental Impact Statement under the National Environmental Policy Act of 1969 (PL91-190). The reasons for the decision not to prepare such Statement are as follows; (a) the review determined that no significant environmental effects will result and (b) the proposed project is consistent with all applicable local, state and federal regulations. An Environmental Review Record has been established for this project that documents the environmental review of the project and more fully sets forth the reasons why such Statement is not required. The Environmental Review Record is on file and available for public examination and copying, upon request, at the office of Hennepin County Housing, Community Works & Transit, 701 Fourth Avenue South, Suite 400, Minneapolis, Minnesota, 55415-1843, telephone 612-348-2599, between the hours of 8:00 a.m. and 4:30 p.m. Monday through Friday. No further environmental review of this project is proposed to be conducted prior to the request for release of funds.
PUBLIC COMMENTS ON FINDING
Any individual, group, or agency may submit written comments on the Environmental Review Record for consideration by Hennepin County to the office of the undersigned. Such written comments will be accepted until May 3, 2013. All such comments so received will be considered and Hennepin County will not request the release of Federal funds or take any administrative action on the project prior to the date specified in the preceding sentence.
RELEASE OF FUNDS
Hennepin County, as the eligible grantee, will provide financial assistance to the project described above with HUD funds from the HOME Investment Partnership Program funds under Title II of the National Affordable Housing Act as amended. Hennepin County certifies to HUD that David Hough, in his official capacity as County Administrator, consents to accept the jurisdiction of the federal courts if an action is brought to enforce responsibilities in relation to environmental review process and that these responsibilities have been satisfied. HUD’s approval of the certification satisfies its responsibilities under the National Environmental Policy Act of 1969 and related laws and authorities, and allows Hennepin County to use HOME funds.
OBJECTIONS TO RELEASE OF FUNDS
HUD will accept objections to its release of funds and the Hennepin County certification for a period of fifteen days following the anticipated submission date or its actual receipt of the request (whichever is later) only if they are on one of the following bases: (a) the certification was not executed by the Certifying Officer of Hennepin County; (b) Hennepin County has omitted a step or failed to make a decision or finding required by HUD regulations at 24 CFR Part 58; (c) the grant recipient has committed funds or incurred costs not authorized by 24 CFR Part 58 before approval of a release of funds by HUD; or (d) another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental quality. Objections must be prepared and submitted in accordance with the required procedure (24 CFR Part 58) and shall be addressed to the HUD grant administration office at 920 Second Avenue South, Suite 1300, Minneapolis, Minnesota 55402. Objections to the release of funds, on the basis other than those stated above will not be considered by HUD. Potential objectors should contact HUD to verify the actual last day of the objection period.

The City Council reserves the right to retain the deposits of the three lowest bidders for a period not to exceed thirty (30) days after the date and time set for the opening of bids. No bids may be withdrawn for a period of thirty (30) days after the date and time set for the opening of bids.
Payment for the work will be by cash or check.
Contractors, subcontractors or material suppliers desiring a copy of the plans and specifications and proposal forms may obtain them at The City of Maple Grove Government Center, Minnesota.
All bidders for this contract, including subcontractors and suppliers which have 40 or more full-time employees, shall submit a certified copy of their current Affirmative Action Declaration with their bid.
The Council reserves the right to reject any and all bids, to waive irregularities and informalities therein and further reserves the right to award the contract to the best interests of the City.
